This form should be printed, filled out, and brought to the school nurse if you would like for your child to take any type of medication while at school.
Medication in the Clinic:
ALL medication must be in the original prescription bottle with the student's name on the prescription label and presented to the nurse within the last three months of the date listed on the bottle. Expired medication and/or expired prescription labels can not be kept in the clinic and will not be administered to students.
PLEASE REMEMBER: NO MEDICATION OF ANY KIND IS TO BE TRANSPORTED TO SCHOOL BY A STUDENT. PARENTS/GUARDIANS MUST BRING MEDICATION TO SCHOOL AND ENSURE A SIGNED CONSENT IS ON FILE IN THE CLINIC. ***IF STUDENT IS REQUIRED TO CARRY AN INHALER FOR ASTHMA, THE NURSE MUST HAVE PRIOR NOTIFICATION***
***Parents should immediately notify the nurse of any new allergies or medical conditions that should be monitored at school.***
***Any student with a seizure diagnosis is required to have a Seizure Action Plan signed by his/her physician in place at the start of each school year as required by law. Contact your school nurse for further information***
Flu Season is Upon Us!
Please remember that your student must be without fever for a full 24-hours without the use of fever-reducing medicine (Tylenol, Advil, Motrin, etc.) before they can come back to school after being sick!
